Case Management Services has a mission to ensure that all eligible persons with developmental disabilities receive the services that assures them the highest quality of life possible.

Case Management is a resource and referral agency that links individuals with disabilities with support and benefits. It is a coordinated delivery of services provided through the Developmental Disability Program. Case Management services are provided to any Montana resident with developmental disabilities who is at least 16 years old. There are programs to help support people with disabilities at work, home, or any other area of their lives. All recipients of the Developmental Disability Program must have a case manager. Although case managers do not provide direct care services, they can assist with the eligibility and referral process to access those services.

Case Management Eligibility

Eligibility must be established through the Developmental Disability Program and Case Management can help with the process of determining eligibility. There is not cost for Case Management services. Services are funding through state and federal governments.

Transition Services

A case manager can be contacted after a student has turned 16. The student should contact their Individual Education Plan (IEP) coordinator to let them know they'd like to meet with a Developmental Disability case manager. If the student already receives case management though a children's mental health or Developmental Disability children's waiver, the case manager assigned to the student will help in the transition from children's to adult services.
