Case Management

Case Management is a resource and referral agency. It links individuals with supports and benefits as well as coordinates the delivery of services provided through the Developmental Disability Program. Case Management services are provided to any Montana resident over the age of 16 with a developmental disability.

If you, or someone close to you, have a developmental disability, there are programs to help support you at work, home and in other areas of your life. A case manager can meet with you and your family to assist in identifying which programs and supports would be most beneficial. Although case managers do not provide direct care services, they can assist with the eligibility and referral process in order to access direct care services.

In many cases, supports and benefits can be accessed without a case manger. However, the Developmental Disability Program requires all recipients of their adult services to have a case manager. This person is in charge of coordinating the plan of care.

The Mission of Case Management:

The mission of case management services is to ensure that all eligible persons with developmental disabilities receive the services that will allow them to live their lives in a way that assures them the highest quality of life possible.

Am I eligible for the Case Management Program?

Any Montana resident, 16 years of age or older, who has a developmental disability* is eligible for case management services. Eligibility must first be established through the Developmental Disability Program and case management can assist with that process.

How much does it cost?

There is no cost to the person or their family. Case management services are an entitlement for all eligible adults and are funded through the State of Montana and the federal government.

Are there waiting lists?

There are no waiting lists for case management services. They are an entitlement for adults with an eligible developmental disability diagnosis. However, there may be a short waiting period while eligibility is determined. Direct care services such as vocational, residential or other supports that are provided through the Developmental Disability Program, Vocational Rehabilitation or other programs often have a waiting list. Sometimes the wait can be several years, other times it can be much shorter. Because funding for the services is limited, it is difficult to estimate how long the wait will be. Frequently, availability of services occurs when someone leaves or gives up their cost plan for services. A case manager can help you determine which services would be most beneficial and how to access those waiting lists.

Transitioning from high school to adult services:

When a student turns 16, a case manager can be contacted. The case manager can meet with the student and their Individual Education Plan (IEP) team in order to assist in the transition planning portion of the IEP. Contact the IEP coordinator at the school and let them know you would like them to invite a Developmental Disability case manager. See below for our offices and the counties they serve.

Sometimes a student will already be receiving case management through another program such as children's mental health or Developmental Disability children's waiver. In those cases, the current case manager will continue to work with you and assist in the transition from children's to adult services. If you receive children's services you can contact your provider to see if this is the case.

How do I get a Case Manger?

To find out if you or a family member is eligible for services, contact the HI Case Management office for your county. If you do not live in one of the counties served by our offices, contact the Developmental Disability Program 406-444-2995, to find the case management office which serves your county.
